01 Modified Fisher Grade

Evaluate: (1) presence of SAH, (2) dense clot formation >1 mm in coronal plane or >3×5 mm in axial plane, (3) intraventricular bleeding.

0 Grade 0 — No subarachnoid or intraventricular hemorrhage 1 Grade I — Focal or diffuse SAH, no clots and no intraventricular hemorrhage. Low vasospasm risk. 2 Grade II — Focal or diffuse SAH, no clots, with intraventricular hemorrhage. Intermediate vasospasm risk. 3 Grade III — Focal or diffuse SAH, with clots, no intraventricular hemorrhage. Intermediate vasospasm risk. 4 Grade IV — Focal or diffuse SAH, with clots and intraventricular hemorrhage. High vasospasm risk.
